In light of the Information Kiosk kindly supply me the following information:
 
1.
Please provide me a small hand drawn sketch showing the location of the Information Kiosk in the hospital.

2.
Contact details of the Dr. Monika Juneja, person-in-charge of the Information Kiosk in the hospital.

3.
Certified copies of any record/document/letter/circular etc. whereby the Information Kiosk have been closed/shutdown or sent for repair/up-gradation etc.

4.
Kindly allow me inspection of the Information Kiosk. Also allow me to take digital pictures of the Functional Information Kiosk at the time of inspection.

5. 
Kindly allow me inspection of all files/records/documents, etc., related to the installation of Information Kiosks as well as the decision leading to installation of the kiosks.

6.
As per Section  5(1) of the RTI Act, every Public Authority has to appoint a CPIO.
Please inform me the Name, Address and Office Telephone numbers of the CPIO. Is this information displayed on your website? If yes, please provide me a certified copy of the print out of that particular webpage where such information has been displayed. If No, please provide me reasons for not displaying this important information as mandated under the RTI Act 2005.

1. The information kiosk is located at New OPD Building, IIIrd floor, D-wing, Room No. 368.

2. Dr. K. M. Agrawal (HOD Psychiatry) is person-in-charge (Nodal Officer) of the information kiosk, and the contact nos. are 26707308 and 26198481.

3. It is incorrect. Information kiosk is still functional.

4. This is to be decided by authorities, i.e., Dte. G.H.S./MoH & FW.

 Reference to your letter dated 11-05-2009 received on 14-05-2009 regarding information requested under Right to Information Act. The required information is as under:-

1. Information kiosk is installed in the OPD Corridor of Deptt of Psychiatry (Barack No.5) RML Hospital, Park Street, New Delhi. This can be easily around Mother Teresa Crescent.

Information kiosk has not been working for some time. Communication has been sent to National Trust for the Welfare of person with Autism, Cerebral Palsy, Mental Retardation and Multiple Disabilities (Ministry of Social Justice and Employment, Govt. of India, 9th floor, Jeevan Prakash building, 25 Kasturba Gandhi Marg, N.Delhi-110001) for its repair.

4. Requests made in these serial numbers do not fall under the definition of 'information' as contained in Section 2(f) of RTI Act. However, you may visit the hospital.

5. The concernment CPIO is Head, Deptt. of Psychiatry, Dr. Ram Manohar Lohia Hospital (Address, Telephone no. are already mentioned in reply Q.No.2). In connection with the website, it is to inform you that the same is under reconstruction and development. This information will definitely be displayed in the website when the website is fully developed.

In reference to your request dated May 13, 2009, fopr information under the above Act, addressed to the Medical Superintendent, AIIMS and his letter reference No. F.1./RTI/09-Estt.(H.) dated 18 May 2009, question wise reply is given below:-

1. The information kiosk is located in the Room No. 14, Department ot Physical Medicine and Rehabilitation (PMR), AIIMS, New Delhi. Sketch is not available on records.

2. Mrs. K B Singh, Chief, MSSO, Department of PMR, AIIMS, New Delhi- 110029-02

3. The information kiosk is functional. It is not closed or shut down or sent for repairs etc. Hence, no records attached.

4. Inspection can be done by fixing prior time during working hours of OPD. For taking pictures, separate permission from appropriate authorities as per AIIMS rules needs to be taken.

5. Records are available in the department. Mutually convenient time may be please fixed for inspection.
